ABOUT THE ROLE

The Community Mental Health Team provides a comprehensive range of services designed to support recovery and promote sustainable community living for people experiencing mental illness.
The Community Mental Health CORE Team is a multidisciplinary team delivering specialist services to adults living in the community. The CORE Team sits within a broader Community Mental Health Network, which includes a specialist Aboriginal Mental Health Team, bilingual mental health workers, and Peer Support Workers, offering a culturally inclusive and person-centred approach to care. This position is located at Canterbury Community Health Centre, Canterbury Hospital.
The Community Mental Health Team (CORE Team) is the mainstay of the Community Mental Health System. The multidisciplinary team includes Nurses, Occupational Therapists, Social Workers, Psychologists, Psychiatrists and Peer Support Workers. Together, we provide a range of clinical services, including intake, comprehensive assessment, care coordination, consultation, and tailored treatment for priority adult populations with mental illness.

“The Clinical Nurse Consultant (CNC) is integral to delivering recovery-oriented and high-quality Community Mental Health care. The key responsibilities of this dynamic role include:

  • Provision of high level mental health nursing skills and capabilities
  • Expert Care Coordination for consumers experiencing complex mental illness living in the community
  • Leadership and innovation to ensure provision of best clinical practice within the multidisciplinary team
  • Contribution to and leadership of the development and delivery of education to students and junior clinicians
  • Participation in formal processes for the strategic and operational planning for the clinical service”
