ABOUT THE OPPORTUNITY

South Western Sydney Local Health District (SWSLHD) Aboriginal Chronic Care Program a dedicated, multidisciplinary team committed to supporting Aboriginal people impacted with renal disease. This team will deliver care through a culturally responsive, holistic model that recognises the importance of connection to culture, community, and country in the healing journey.
We are currently seeking a Clinical Nurse Specialist Grade 1 to join this innovative team. This role offers a unique opportunity to work alongside a passionate group of professionals—including Aboriginal Health Workers, Counsellors, a Social Worker, and an Aboriginal Program Coordinator—to provide person-centred care that is grounded in cultural safety and respect. The successful candidate will work across acute and primary care settings, supporting client engagement, care planning, care coordination, and harm reduction treatment.

This position is based in Miller and will involve travel across the South West Sydney region, working on the lands of the Dharawal, Darug, and Gundungurra peoples. As part of the Aboriginal Chronic Care Program, you will be supported by an Aboriginal Program Coordinator and have access to a wide range of cultural supports, including:

• Cultural supervision and mentoring
• The SWSLHD Aboriginal Health Unit
• A strong network of Aboriginal colleagues across the district

The Clinical Nurse Consultant (CNC) is an expert practitioner who, through leadership facilitates evidence based practice. The CNC will promote a service which fosters clinical excellence, research and professional leadership. The CNC contributes to quality health care by collaborating with the health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of the care in the given specialty.
