In this position you will:

• Co-ordinate the patient’s process through the hospital system by providing complex case management, facilitate timely access to care and support discharge planning.
• Recognise deteriorating/unstable patients based on verbal and written clinical handovers (including diagnostic tests, ie bloods, ECGs, etc) and liaise with Patient Flow Unit on the priority and plans of Inter-hospital transfers.
