Responsibilities

KEY RESPONSIBILITIES:

You will operate within a criteria-led discharge framework and assume leadership in the following domains:

  • Clinical Review: Apply pre-established discharge criteria to assess patient readiness for discharge
  • Coordination: Collaborate with multidisciplinary teams to finalise discharge summaries, prescriptions, and follow-up arrangements
  • Communication: Liaise effectively with patients and families regarding discharge plans and expectations
  • Documentation: Ensure accurate and comprehensive recordkeeping to support safe transitions and quality assurance
  • Decision-Making: Exercise professional judgment within scope of practice, with strong attention to clinical detail

The role offers enhanced autonomy within a structured model, opportunities for professional development, and visibility across departments. This is a short-term secondment / temporary contract position, subject to ongoing evaluation. Extension may be considered pending outcomes and organisational review.

Deliver and evaluate clinical education programs in consultation with Management, Nurse Educators and Clinical Nurse Consultants, to support the delivery of clinical best practice, patient service and enhanced patient outcomes.
