TCP Homecare are a leading provider of homecare nursing services on a national scale.
We are looking for an innovative & enthusiastic professional, who demonstrates a high degree of self-motivation, to lead a nationwide service within the home and community setting.

KEY RESPONSIBILITIES:

Administration of sub-cut injections of the Product in the hospital clinic setting, home environment or providing support via virtual calls.
Monitoring for adverse events and supporting patients with side effect management
Education of Patients on the importance of the Product in controlling their disease.
Liaison with multi-disciplinary hospital team regarding prescribing and treatment
Liaise with multi-disciplinary hospital team and patients to co ordinate scheduling of patient visits
Service coverage of identified Hospital within the Republic of Ireland
Reporting of nursing notes as per hospital protocol
Maintaining positive and constructive relationships with client and HCPs
Implement all TCP Homecare operating procedures and protocols relating to the care and safety of a patient and yourself.
Ensure a safe clinical environment for patients and yourself by following all procedures and guidelines.
Attend relevant meetings with company and/or clients and facilitate meticulous feedback.
Participate in approved data collection, clinical studies and trials as requested by the client.
Provide support and advice to patient and family member’s clients and multi-disciplinary team.
To attend all company required meetings, training and other business in a prepared and punctual manner
Prepare monthly reports on service activity
Prepare and deliver PowerPoint presentation of service activity as required
Partake in regular service review meetings with client
