As the Clinical Nurse Leader, you will be responsible for ensuring a consistently high quality of care is provided to our residents, to facilitate and support their continued enjoyment of life within their level of capability. Integral to this is proactive management of resident care, commitment to quality assurance, continuous improvement and high standards of work ethics, whilst ensuring compliance to Bolton Clarke guidelines and policies.
The role requires strong relationships to be maintained with residents and their families and friends, care staff, leisure and lifestyle coordinators and external health & allied professionals.
This role will primarily cover weekends, with a minimum 30 hours per fortnight.

Responsibilities
Delivering a high quality of care to each resident in the home, respecting their privacy and dignity while at all times displaying quality customer focus.
Develop, monitor and implement the nursing care plan as designed for each resident by the health care team and the resident / representative, making changes based on clinical assessment and liaising with resident/family regarding care requirements.
Assist in the assessment, planning and evaluation of care for residents with complex needs
Perform comprehensive assessments and develop short and longer term treatment plans in line with diagnostic findings
Lead case conference approaches ensuring all relevant input is evaluated and considered in planning and providing person centred care
Drive strong clinical governance across the organisation and acts as an advocate for residents and their families in influencing clinical policy
Participate in clinical research and evaluation initiatives
Provide direct and indirect supervision for registered nurses, enrolled nurses and care staff, including acting as a point of contact for escalation of challenges
Co-ordinating the day to day management of the Home in the absence of the General Manager and Care Manager (as requested)
