Essential Duties and Responsibilities: This section describes the essential functions of this role; however, this is not intended to be all-inclusive. Other duties may be assigned, as necessary. Patient Care Coordination: Ensures promotion and control of optimal patient care using outcome based and best-practice care planning. Receives case referrals from the Intake Department for review to determine appropriateness of services and if the required skill set is available in the patient territory. Assists Intake and Care Coordinators with assigning the appropriate clinicians to cases. Reviews patient’s clinical diagnosis, medications; oversees the assessment (SOC), assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care (POC). Triages patient care events, initiates the proper protocol, collaborates and/or delegates actions to the clinical operations and/or clinical coordination teams. Ensure timely and accurate documentation of patient care activities and progress. Facilitates case conference meetings with interdisciplinary teams to facilitate coordination of care and make necessary adjustments to the plan of care (POC) as appropriate. Coordinate with physicians, patients, and their families to develop and adjust care plans as needed. Maintains logs including but not limited to Admissions, Discharge, and Infusion; tracks details to determine appropriate next steps (Discharge, Evaluation, Recertification or Resumption of Care) Responsible for physician orders management; review and approval of orders Clinical Supervision: Provide leadership and support to clinical staff including; Registered Nurses, Licensed Vocational Nurses, Medical Social Workers and Home Health Aides. Works closely with the Director of Rehabilitation who has oversight of the therapy team. Conduct regular performance evaluations, offering constructive feedback and identifying opportunities for professional development. Facilitate ongoing education and training to ensure staff competency and compliance with best practices and regulatory standards. Serves as a resource and mentor to clinical staff, offering clinical guidance and support as needed. Quality Assurance: Monitor patient outcomes and satisfaction, implementing improvement plans when necessary. Ensure compliance with all federal, state, and local regulations, as well as accreditation standards. Conduct regular audits of patient records and clinical practices to maintain high standards of care. Reviews Home Health Resource Group (HHRG), Low Utilization Payment Adjustment (LUPA) rates in order to ensure proper utilization of services. Performs audits including but not limited to; insurance based audits. Operational Management: Assist in the recruitment, onboarding, training and retention of qualified clinical staff. Works closely with the Talent Acquisition team to ensure the proper level of staffing is maintained at all times. Manage staff schedules to ensure adequate coverage and continuity of care. Address and resolve any issues or conflicts that arise within the clinical team or with patient care. Ensure that supplies are allocated appropriately to remove wastes and contain costs, processes requests in a timely manner to avoid delays (review escalated supply reviews) Assists Clinical Director/Director of Patient Care Services in the planning, implementation and evaluation of inservice and continuing education programs. Assists Clinical Director/Director of Patient Care Services in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives. Complies with accepted professional standards and principles. Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system. Promotes customer service orientation to all organization personnel. Reviews operational processes and presents recommendations in order to improve field clinician efficiency and satisfaction Assist with regulatory surveys and audits, providing documentation and support as needed. Stay informed about changes in regulations, industry standards and changes in policies and procedures Collaboration and Communication: Foster a collaborative environment among the healthcare team to promote effective communication and teamwork. Serve as a liaison between clinical staff, operational staff, and external stakeholders. Participate in multidisciplinary meetings to discuss patient care plans and progress. Communicate effectively with patients and their families, addressing any questions or concerns related to clinical care. Maintain open lines of communication with the clinical leadership team to report on clinical activities, outcomes, and challenges. Responds to patient complaints, follows up as necessary with clinicians, escalates to leadership as appropriate and completes documentation of events. Requirements Current and unrestricted CA Registered Nurse (RN) Graduate of National League for Nursing accredited school of nursing. BSN degree is preferred. 1-2 years of direct field nursing experience within the Home Health setting. Knowledge and expertise in clinical decision-making and Quality Management review and evaluation. Complies with accepted professional standards and practice. Complies and maintains current CPR certification.
