Your role at St John of God Social Outreach We are looking to hire Mental Health Clinical Nurses to join our Medicare Mental Health Service in Armadale, on a permanent full-time basis. Assist in the planning, implementation and evaluation of the day to day mental health and basic physical care requirements of patients. Participate in a variety of mental health and other clinical assessments, risk assessment and management; history taking and developing comprehensive care plans. Our team provides a safe and welcoming spaces for locals to walk in and access free mental health support and services - no referral, Medicare card or appointment needed. Staffed by qualified mental health professionals and people with lived experience, the centres provide immediate support to adults in distress, including their families, friends and carers. We can also connect people with other support services, such as employment assistance, to support them in the future. The Position Applies advanced clinical knowledge and skills in the coordination of direct care to consumers. Undertakes comprehensive Mental Health Assessments and develops care plans in collaboration with the multidisciplinary team. Acts a clinical resource, maintains and develops clinical skills relevant to area of practice. Initiates reviews and coordinates care of consumers with complex mental health problems. Provides brief, individual therapeutic interventions (up to six sessions) where indicated, and co-facilitates psychoeducational groups. Serves as a consumer advocate, protecting the rights and safety of consumers at all times. Evaluates the quality of nursing practice and participates in quality improvement initiatives to improve consumer outcomes. To be successful in this role, you will be a Registered Nurse with current registration with the Nursing & Midwifery Board of Australia (AHPRA) and have relevant post-registration experience within an acute mental health setting. Successful applicants must hold a current Working with Children check and National Police Clearance.
