Your role at St John of God Social Outreach We are seeking to hire a Clinical Nurse Mental Health to join our Medicare Mental Health Centre (Mirrabooka/Balga) on a permanent full-time basis. This role provides a high standard of consumer care, acts as a clinical resource and effectively manages resources to deliver quality consumer outcomes in the relevant Medicare Mental Health service setting. You will assist in the planning, implementation and evaluation of the day-to-day mental health and basic physical care requirements of patients. Participate in a variety of mental health and other clinical assessments, risk assessment and management; history taking and developing comprehensive care plans. The Position Applies advanced clinical knowledge and skills in the coordination of direct care to consumers. Undertakes comprehensive Mental Health Assessments and develops care plans in collaboration with the multidisciplinary team. Supervises nursing practice and takes responsibility for standard of care delivered. Acts a clinical resource, maintains and develops clinical skills relevant to area of practice. Initiates reviews and coordinates care of consumers with complex health problems. Serves as a consumer advocate, protecting the rights and safety of consumers at all times. Evaluates the quality of nursing practice and participates in quality improvement initiatives to improve consumer outcomes. The ideal candidate will be registered with the Nursing & Midwifery Board of Australia (AHPRA) as a Registered Nurse and have relevant post-registration experience in an acute mental health environment. Successful applicants must have a valid Working with Children’s Check and National Police Clearance.
