The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
NBMLHD have on offer an exciting opportunity for a Clinical Nurse Specialist to join our Planned Care for Better Health Team as Care Coordinator. This position is a permanent full time position working out of either Lithgow, Lawson or Lemongrove Community Health Centres.
The NBMLHD Planned Care for Better Health team provide comprehensive multidisciplinary assessment and time limited intervention to ensure clients with chronic conditions, complex social needs and those at risk of hospitalisation are provided with coordinated and timely care. The primary purpose of the CNS2 Care Coordinator is to work collaboratively with the client and relevant health providers across the Nepean Blue Mountains Local Health District (NBMLHD) to enable clients to self-manage their own health care. Your role will provide care navigation, care coordination and health coaching to identified clients. As Care Coordinator you will develop and maintain good working relationships with General Practitioners, health care providers and other relevant stakeholders.
