EXCITING OPPORTUNITY – CLINICAL NURSE SPECIALIST GRADE 2 (DISCHARGE PLANNER)

Are you a motivated and experienced Registered Nurse with a passion for patient-centred care and safe, timely discharge planning? We are seeking a dynamic Clinical Nurse Specialist Grade 2 – Discharge Planner to join our Medical Ward team. This advanced practice nursing role provides leadership and clinical expertise in discharge coordination, working collaboratively within a multidisciplinary team to ensure safe, effective, and person-centred transitions of care. The successful applicant will hold relevant post-registration qualifications with at least three years’ experience in the clinical specialty or such other qualifications deemed appropriate by the organisation, and will demonstrate high-level autonomy, expert clinical judgement, and an ability to navigate complex discharge pathways. As a CNS2, you will contribute to clinical practice development, mentor others, and champion excellence in discharge planning aligned with NSQHS Standards 2, 5, and 6, the Admission to Discharge Care Coordination policy (PD2022_012), and MNCLHD performance targets. You’ll be instrumental in driving improved patient flow, reducing avoidable delays, and delivering safe, coordinated discharges that meet the needs of patients and their families.
