ABOUT THE ROLE

An exciting opportunity exists to join our Western Virtual Patient Flow Access & Command Centre.
Delivering a service across many locations, the Clinical Nurse Specialist 2 will partner with our existing Patient Flow, Inpatient and Connected Care in Community teams, supporting discharge coordination processes to meet key performance outcome measures.
This position is a rotating roster, morning and afternoon shifts, 7 days a week, supporting a coordinated multi-disciplinary team approach.
