HammondCare at Home is currently looking for an experienced Clinical Partner to support the team on the Far North Coast. What is a Clinical Partner? The Clinical Partner works within HammondCare’s Mission in Action to deliver high-quality, person-centred clinical care that supports clients to live well in their own home. The role provides evidence-based, best practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed. This role is focused on assessing clinical care needs, developing, and reviewing care plans and delivering excellent clinical care in partnership with clients and key stakeholders. The role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care. Education, Experience and Skills Essential/Mandatory: Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action. Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General). A current driver’s license and the ability to travel between HammondCare’s sites and services. Passion for and previous experience in client-focused service delivery in aged and community/home care settings. Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards. Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care. Ability to work effectively in a multidisciplinary team and independently across multiple sites. Excellent administrative and computer skills, including digital health records and care coordination tools. Empathy, patience, and a passion for delivering high-quality, person-centred care. Desirable: Nursing experience in aged care and community care. Ability to identify and implement innovations in care delivery and service improvement.
