Responsibilities
  • Collaborate with physician specialists to initiate and monitor treatment plans.
  • Coordinate with Patient Support Programs and dispensing pharmacies to streamline drug access and follow-up care.
  • Assist with documentation related to drug access navigation, including medical clearance forms and standing orders where applicable.
  • Participate in interdisciplinary case conferences to align treatment plans across multiple providers.
  • Use Electronic Health Record (EHR) systems to document clinical findings and identify relevant clinical factors tied to the patient’s treatment plan.
  • Review medical histories in relation to the patient’s primary concerns and ongoing conditions.
  • Provide education to patients regarding treatment plans, medication use, and follow-up expectations.
  • Maintain confidentiality and safeguard patient health information in accordance with professional standards.
  • Contribute ideas and feedback to help improve Treo’s care coordination model as we continue to grow.
