One Senior Care is seeking a dynamic and experienced Clinical Pharmacist to serve as a senior leader across our contracting and clinical pharmacy functions. This role is critical in guiding formulary and operational decisions, especially those related to risk management and Medicare Part D. The ideal candidate is a strong, strategic leader with demonstrated expertise in pharmacy/PBM bidding, EHR and EDS conversions, and advanced medication management for older adults.
As a key member of our interdisciplinary healthcare team, you will ensure optimal medication therapy, regulatory compliance, and cost-effective care. You will collaborate with internal and external stakeholders, lead pharmacy operations, and drive continuous improvement initiatives that enhance participant outcomes.

QUALIFICATIONS

• Active Pharmacist license and Controlled Substance license in the states of practice, with a collaborative practice agreement.
• Doctor of Pharmacy (PharmD) required; PGY1 Residency in Ambulatory Care or Geriatrics preferred.
• Board Certification in Geriatric Pharmacy or Pharmacotherapy is encouraged.
• Demonstrated experience in senior care, long-term care, or PACE environments preferred.
• Proven leadership experience in pharmacy/PBM bidding, EHR and EDS conversion, and medication management.
• Deep understanding of formulary management, operational risk, and Medicare Part D.
• Strong negotiation, communication, and collaboration skills.
• Proficiency with electronic health records and medication management systems.
• Commitment to ongoing professional development and best practices in geriatric pharmacotherapy.

Leadership & Operations

• Lead and oversee all pharmacy-related contracting, including PBM negotiations, formulary management, and pricing strategies.
• Direct operational initiatives related to pharmacy services, including EHR and EDS conversions, ensuring seamless integration and regulatory compliance.
• Make informed, strategic formulary and operational decisions to manage organizational risk, especially in the context of Medicare Part D.
• Develop and implement medication management policies, order sets, and clinical practice guidelines.

Participant Care

• Conduct comprehensive assessments to inform care planning, transitions, and discharge processes.
• Review and monitor medication regimens for safety, effectiveness, and appropriateness, with an emphasis on polypharmacy and chronic disease management.
• Communicate medication changes and updates to the care team, ensuring accurate and timely documentation.
• Counsel participants, families, and caregivers on medication use, side effects, and adherence strategies.
• Educate staff, students, and healthcare professionals on medication best practices and safety protocols.
• Promote a positive and supportive participant experience at every touchpoint.

Medication Management & Compliance

• Lead comprehensive medication reviews and utilization initiatives, including regular regimen reviews as required by law.
• Ensure compliance with Medicare Part D and other regulatory requirements.
• Identify and resolve drug-related problems, recommend therapy adjustments, and implement cost-effective medication strategies.
• Monitor prescribing trends, implement pharmacy initiatives, and stay current with clinical literature and best practices.
• Perform audits, support leadership with reporting, and assist with data collection for regulatory and quality pharmacy initiatives.
• Manage the medication formulary and participate in preferred medication programs.

Collaboration & Quality Improvement

• Work closely with hospitals, pharmacies, and external providers to ensure seamless care transitions and close care gaps.
• Participate in interdisciplinary team meetings, committees, and quality improvement projects.
• Contribute to the education and competency assessment of staff.
• Utilize pharmacogenetic testing and advanced medication management tools as appropriate.
