The Clinical Pharmacist – Patient Care Management Hub contributes to AHF by delivering direct and remote patient care and medication oversight, fostering enduring patient-provider relationships, coordinating care efforts, advocating for patients, promoting wellness, promoting adherence to their overall care plan, facilitating patient education, and self-management by supporting multiple locations including but not limited to Pharmacy4Humanity and AHF pharmacies. Responsibilities include, but are not limited to, performing data entry for patient information and medications while ensuring compliance with HIPAA regulations, overseeing claim processing for third-party programs as well as centralization of prior authorizations, billing of clinical services, review/verification of patient medication regimens, retention and re-engagement of patients, and addressing any adherence issues. This pharmacist will also be responsible for reviewing and completing various pharmacy reports. The Clinical Pharmacist - PCM enhances the reputation of AHF by enriching the quality of patient care, improving patient satisfaction, and achieving positive health outcomes though personalized care. The Clinical Pharmacist – PCM will work as a part of a close-knit team to provide safe and efficient delivery of pharmaceutical services of a Patient Care Management Hub according to AHF pharmacy policies and Standards of Practice. Licenses and Certifications: State license for the location you are applying for, one additional State license and will be required to obtain AAHIVP Certification within one year of hire.
