Note: Eligibility for benefits may vary by location and is determined by employment status Summary: Antimicrobial Stewardship Pharmacist Specialist Review and assessment of key antimicrobials prescribed and microbiologic cultures generated at the designated site Provide interventions (or oversight thereof) to promote safe, cost effective and rational antimicrobial use for improved patient outcomes as directed by the Antimicrobial Subcommittee (ASC) Documentation of interventions as described above Oversight of Criteria Monitored Drug (CMD) process for antimicrobials at specific hospital site Oversight of pharmacokinetic (PK) dosing service related to antimicrobials including daily review of PK data (or oversight thereof) of all patients receiving aminoglycosides and/or vancomycin. Provide training to clinical pharmacy staff as needed. Provision of drug information related to antimicrobial issues to health care professionals at the site as identified by the ASC 1. Doctor of Pharmacy degree (with residency or fellowship preferred) or equivalent combination of education with one to two years of clinical pharmacy practice experience in a health care setting. 2. Registered Pharmacist and Controlled Substance licensures in the State of Michigan obtained within 3 months of entry into job. 3. PGY2 residency training in ID is preferred but not required. 4.
