Description
The Clinical Pharmacist Specialist is responsible for the provision of advanced Clinical Pharmacy services within acute and ambulatory care settings including, clinical interface with physicians and nurses, process improvement and medication therapy management. Reviews medication orders for compliance with aspects of Pharmaceutical Care. Provides patient education regarding medications. Contributes to specialty practice through committee participation and development of policies, procedures, and pharmacy practice protocols. Trains pharmacy students and pharmacy practice residents. Trains pharmacist staff in areas related to acute and ambulatory care. Directly contributes to patient care through the coordination of departmental clinical practices and programs. Provides in-service education and drug information to health care professionals. Education Bachelor's Degree Pharmacy or PharmD Required Other PGY1 Pharmacy Residency or a relevant combination of education and experience Required Experience 1 year experience in institutional practice or residency Required Licenses PA Pharmacist License Required Cert Pharmacotherapy Spec Preferred
