Responsibilities:
ABOUT THE ROLE:
As a Clinical Pharmacist Specialist, you will be at the forefront of delivering optimal medication therapy for specialized patient populations. You will work closely with physicians, nurses, and other healthcare team members to ensure comprehensive and individualized clinical pharmacy services. Your expertise will be crucial in integrating clinical pharmacy into the care process and improving patient outcomes.
This role will mostly work day shift, but may work some evening/night hours as needed. This role also requires working rotating weekend and holiday shifts.
KEY RESPONSIBILITIES:
- Lead and Innovate: Oversee patient service lines, clinical areas, and therapeutic programs.
- Patient Care: Provide direct patient care, including medication review, reconciliation, and counseling.
- Collaboration: Work as an active member of a multidisciplinary team to deliver patient-centered care.
- Quality Improvement: Identify practice-related issues and facilitate clinical research projects and quality improvement initiatives.
- Education and Mentorship: Serve as a preceptor for pharmacy students and residents, and develop training experiences.
- Operational Excellence: Maintain proficiency in decentralized pharmacy services and clinical programs, and oversee pharmacy operations.
