POSITION SUMMARY:

Reporting to the Senior Manager of Pharmacy, the Clinical Pharmacist works according to current Ontario College of Pharmacists’ Standards and all relevant legislation, to promote the safe and appropriate usage of medications for inpatients at our main site. As part of an interprofessional team, the successful candidate will provide direct patient care in the management of drug therapy to achieve positive patient health outcomes.
The pharmacist will keep abreast of current information and changes in official regulations (including changes to scope of practice) through continuing education and official publications.

QUALIFICATIONS AND EDUCATION:

  • Licensed to practice pharmacy in Ontario as Part A Registered Pharmacist (RPh)
  • Bachelor of Science in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD)
  • Knowledge of Provincial and Federal legislation governing all aspects of drug use.
  • Minimum two years relevant experience in hospital pharmacy practice preferred.
  • Commitment to continuous improvement and learning.
  • Possess resilience, adaptability, initiative and strong communication and interpersonal skills to effectively facilitate the delivery of pharmacy services
  • Highly motivated, compassionate individual able to work closely with patients and their family members, physicians, nurses, and other Allied Health professionals using an interprofessional approach to providing patient care
  • Knowledge of drug distribution systems, including unit dose and automated dispensing cabinets.
  • Excellent attendance record.
  • High degree of accuracy and attention to detail.
  • Ability to meet bona fide occupational requirements as outlined in the Job Demands Analysis.

Responsibilities
  • Facilitate and promote accurate, safe, appropriate and cost-effective use and distribution of medications throughout the hospital
  • Review medication orders including conducting Medication Reconciliation on admission and discharge
  • Provide pharmaceutical care, therapeutic drug monitoring, drug information, patient education and participate in family meetings as required
  • Participate in quality assurance initiatives
  • Support Registered Pharmacy Technicians in procurement, dispensing and non-sterile compounding
