Description

WE ARE LOOKING FOR A CLINICAL PHARMACIST TO JOIN OUR GROWING CLINICAL TEAM.

We are an enthusiastic and forward-thinking practice based in the Rhondda Valley, 30mins scenic drive from Cardiff, based over 2 sites in Tylorstown and Ynyshir.
Our team currently consists of 3 GPs, 2 ANP’s, fully supported by a full multidisciplinary team. Our list size is approximately 6000 patients, and we currently provide a full range of Enhanced Services. We are in the process of becoming a training practice.

What You’ll Do:

  • Work closely with GPs, nurses, and other team members to optimise medicines use
  • Conduct structured medication reviews and manage repeat prescribing processes
  • Support patients in managing chronic conditions through expert pharmaceutical care
  • Contribute to quality improvement projects and promote safe prescribing practices

What We are Looking For:

  • GPhC-registered pharmacist with excellent clinical knowledge and communication skills
  • Experience or interest in primary care, medicines optimisation, and patient-centred care
  • Ability to work independently and collaboratively within a multidisciplinary team
