ABOUT YOU

We’re looking for a motivated and skilled Primary Care Clinical Pharmacist who:

  • Is a GPhC registered Pharmacist.
  • Ideally holds (or is working towards) an Independent Prescribing qualification.
  • Has completed or is undertaking the CPPE Primary Care Pharmacy Education Pathway (PCPEP), or is willing to enrol.
  • Has experience in primary care or community pharmacy (desirable).
  • Possesses excellent communication and clinical decision-making skills.
  • Can work independently and collaboratively as part of a supportive team.

As a valued member of the practice, you’ll play a vital role in ensuring safe, effective, and evidence-based use of medicines. This GP Surgery Clinical Pharmacist role offers you the chance to work at the heart of primary care, improving patient outcomes while developing advanced clinical skills.

KEY RESPONSIBILITIES

  • Conduct structured medication reviews with patients.
  • Support long-term condition management through medicines optimisation.
  • Provide expert advice to GPs, nurses, and the wider team on prescribing.
  • Deliver clinics, including polypharmacy, minor ailment, and medication review clinics.
  • Improve repeat prescribing systems and reduce medicines waste.
  • Contribute to quality improvement and clinical governance activities.
  • Support audits, QOF targets, and patient safety initiatives.
