Description POSITION TITLE: Clinical Pharmacist CLASSIFICATION: Exempt DEPARTMENT: Tribal Health SUPERVISOR: Pharmacy Director or Designee GRADE: E19 $51.93 ($108,021.89) – $77.90 ($162,032.83) Position Summary: This position is under the direct supervision of the Pharmacy Director. The Clinical Pharmacist works independently and, as part of a patient’s care team, is responsible for consulting with and advising Medical, Nursing and provider staff on drug information, recommending patient medication therapy and providing counseling and education to patients at all clinic sites (main, secondary, satellite). Attend patient visits with providers upon request, schedules pharmacy related appointments with patients, provides in-service and education to Medical, Nursing and provider staff, serves as a resource and drug information specialist to Medical, Nursing and provider staff and performs comprehensive clinical assessments and collaborates with the integrated healthcare team to provide quality patient care. Responsible for the provision of safe, effective, appropriate and cost-efficient medication therapy to patients. Contributes to the efficiency and effectiveness of the Washoe Tribal Health Center to ensure that the stated and perceived needs are met. Maintains confidentiality of all privileged information. Essential Duties & Responsibilities: · Provides therapeutic consultations, medication management services, in person and telephone-based therapy management and collaborating on multidisciplinary teams to optimize therapy medication for patients. · Assesses patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response and providing clinical interventions to detect, mitigate and prevent adverse medication events. · Completes critical patient monitoring and reviews patient profile/chart to identify, prevent or mitigate drug related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication and treatment failures. · Collaborates with other healthcare professionals to plan, monitor, review and evaluate the quality and effectiveness of drugs and drug regimens, providing advice on drug applications and characteristics. · Monitors patient lab results in order to see if medications are having a negative effect on any organs or systems. · Performs medication reconciliation. · Provides medication review, reconciliation and counseling as appropriate for patients being discharged from hospital care or transitioned into or out of health systems, incarceration, rehab programs, etc. · Reviews patients’ chart to ensure that prescribed treatment is appropriate for patient diagnosis, comorbidities, allergies and current medication regimen. · Documents all patient consultations, education, reported adverse drug events, teaching sessions and medication reviews in the patient electronic record. · Documents all reviews, recommendations, adverse drug events and interventions performed or found during chart review in the patient electronic health record. · Assesses and document Adverse Drug Events. · Provides counseling and education to patients of all ages. · Explain the instructions, benefits and side effects of medications to patients in a way that will ease any fears or anxieties a patient may have towards taking medications. · Monitors patient medication compliance and over/under usage patterns. · Reviews and interpret prescriptions for patients as an overall check in the schedule of medication dose times, route of administration, correct medication, dose, potential allergies, chemical/physical incompatibilities, drug interactions and drug duplications. · Proper management of patient records and pharmacy files. · Responsible for inputting data into the computer healthcare system, such as prescriptions, notes for provider review regarding medication therapy and any patient counseling done. · Assures safe and cost-effective drug therapy and appropriate outcomes in the use of drugs and controlled substances. · Participates in the WTHC quality management, or quality improvement, program. · Assists with advocacy services and supportive/clinical care while navigating through complex specialty disease states. · Participates in all matters related to pharmacy, including the formulation of policies and procedures. · Assists in formulation, management and upkeep of pharmacy formulary. · Actively identify practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives or healthcare provider education as needed to advance practice. · Ensures pharmacy is an integral part of the healthcare delivery system and facilitate enhancement and expansion of pharmacy services and program. · Provides information and consultative services to the medical staff on drugs, drug characteristics, dosage, indication, contradictions, adverse reactions, allergies and availability of alternative medications, etc. · Responds to and resolve pharmacy related inquiries from clients, providers, staff or pharmacies related to formulary options, therapeutic alternatives and prior authorizations following established protocols and procedures. · Serves as department resource and liaison to other departments, personnel or external groups. · Works in conjunction with physicians, nurses, other pharmacists, other medical personnel, students (pharmacy or medical). · Provides medication and practice related education/training and actively serve as preceptor for doctor of pharmacy students and pharmacy residents. · Communicates with representatives and vendors of drug companies. · Oversees pharmacy operation and technician practice/activities when necessary. · Performs other duties as assigned or required. Requirements Minimum Qualifications: · Bachelor’s or doctorate degree in pharmacy from an educational institution recognized by the American Council on Pharmaceutical Education and 5 years appropriate licensure as a registered pharmacist with 2 years experience in a pharmacy setting. · Specialty certification in ambulatory care or diabetic education in two years from date of hire. Knowledge, Abilities, Skills, and Certifications: · Ability to obtain knowledge of and experience with clinical documentation of pharmacy services in Electronic Health Record (EHR) and pharmacy database systems, with ability to adapt to and learn new systems. · Ability to navigate within the clinic EHR and pharmacy database systems, working directly with appropriate IT staff to resolve any issues/needs/wants necessary to increase efficiency and improve patient care. · Ability to communicate effectively in the English language both verbally and in writing. · Ability to present information (verbal and/or written) in a clear and concise manner that is easy to hear or read and understand. · Active listening skills, good bedside manner, and ability to effectively interview patients to have them talk about their symptoms, barriers to care, past medication reactions. · Knowledge and skills are necessary to provide care appropriate to the age and disease state of the patients being served. · Knowledge to develop and implement medication treatment plans, monitoring patients to ensure safety and efficacy of medication therapy. · Knowledge to develop, implement and monitor patient’s individualized plan of care to include setting of therapeutic goals, monitoring of progress to goals and ongoing communication with prescribing physician. · Knowledge of the practices of other disciplines related to pharmacy. · Knowledge of applicable federal, state and local regulatory laws and regulations including established professional standards. · Knowledge of principles, theory, methods, and equipment involved in the practice of pharmacy. · Ability to interact with and make recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care. · Ability to handle stressful/busy situations, multiple tasks and meet deadlines. · Ability to analyze and problem-solve. · Effective time management and organizational skills. · Knowledge of a Tribal community clinical setting or demonstrates the ability to be sensitive to, and work with, Native Americans. · Ability to instruct members of the clinic staff in pharmacy. · Ability to establish and maintain effective working relationships with patients, medical staff and other personnel. · Skill in operating business computers and office machines in a Windows environment, specifically Microsoft 365, Word, Excel, and PowerPoint; pharmacy software and EPIC EHR system. · Knowledge of modern office practices, procedures, and equipment. · Ability to maintain proficiency in clinic computer systems and medication ordering systems. · Knowledge of business English, proper spelling, grammar, punctuation, and basic arithmetic. · Knowledge of records management procedures. · Ability to represent the organization in a professional manner, building respect and confidence in the Tribal and professional communities. · Ability to obtain Basic Life Support (BLS) certification within six months of hire.
