The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. Required Qualifications: * Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or, * PharmD in PHARMACY from an accredited School of Pharmacy. * Coursework/Training - Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or equivalent education/experience. * Oregon Pharmacist License upon hire. * Certification from Board of Pharmacy Specialties within 2 years of hire. * TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
