DESCRIPTION

The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high-quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas:
(1) Patient Care - The Clinical Pharmacist Specialist is responsible for providing collaborative practice services in collaboration with the healthcare team.
(2) Population Management - The Clinical Pharmacist Specialist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization.
(3) Preceptor / Educator - The Clinical Pharmacist Specialist will provide education to the healthcare team and patients in written and verbal formats that include formal and informal presentations and publications. The Clinical Pharmacist Specialist is a drug therapy resource to physicians and other members of the health care team. The Clinical Pharmacist Specialist may participate in the selection and training of pharmacy learners.

REQUIRED QUALIFICATIONS:

  • Bachelor’s Degree In PHARMACY from an accredited School of Pharmacy. Or
  • PharmD In PHARMACY from an accredited School of Pharmacy.
  • Upon hire: Washington Pharmacist License.
  • Within 2 years of hire: Board Certified by the Board of Pharmacy Specialties specific to area of practice within 2 years of starting the role.
  • Upon hire: Pharmacy preceptor license in applicable state as applicable.
  • 2 years TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.

PREFERRED QUALIFICATIONS:

  • Coursework/Training Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience. Or equivalent educ/experience.
