Job Description:
The Clinical Practice Leader (CPL), Inpatient Diabetes works with the leadership team within the clinical area and is responsible for provision of visible clinical support, coaching and modeling to ensure that the care provided in the hospital setting meets current professional and health care standards. The incumbent will work in collaboration with the interprofessional team to facilitate optimal patient care through education, providing resources and protocol development in a way that is consistent with Mackenzie Health’s vision, mission and strategic direction. They will serve as a bridge between the inpatient/acute care setting and the outpatient Diabetes Education Centre. The incumbent will support the new CVH Hospital while working collaboratively with the CPL Diabetes incumbent at the Richmond Hill site.
