We have a dedicated team working in our virtual ward medical hub in a multi-professional team. We have an ambitious aim for our NHS@Home Virtual Ward pathways to be truly integrated between primary, community and secondary care and to offer a high quality, alternative to acute bedded care for the people of Gloucestershire. These roles offer an exciting new opportunity for skilled nurses and AHPs to develop the service with our dedicated medical and community teams and to gain experience working at the interface between community and secondary care for acutely unwell patients.

The service will aim to manage patients safely at home with a range of medical conditions who would otherwise be in hospital such as respiratory, frailty, heart failure – with the support of specialty teams and our digital partner we believe that this is an exciting time to be joining this new team.
