Clinical Quality Coordinator – Multi-Specialty Internal Medicine Clinic Certified Medical Assistant with primary care or internal medicine experience needed. This is a non–patient-facing role performed within the clinic focusing on chart review, data quality, and closing care gaps (e.g., mammograms, colonoscopies, bone density). JOB SUMMARY The Clinical Quality Coordinator is responsible for promoting high-quality patient care and service delivery by proactively identifying and addressing care gaps, conducting patient outreach, coordinating necessary testing and follow-up appointments, and supporting clinical workflows. This role also involves identifying opportunities for improvement and leading the development and implementation of quality improvement initiatives to enhance patient outcomes and operational efficiency. EXPERIENCE AND EDUCATION Required Education Graduate of an accredited LVN program or Certified Medical Assistant School Experience 2 years JOB DUTIES Identifies patients with care gaps and collaborates with care teams to address those gaps. Conducts patient outreach to coordinate follow-up visits and close care gaps. Arrange laboratory or imaging orders based on identified care needs and clinical protocols. Performs low-complexity clinical procedures such as visual acuity testing, developmental and musculoskeletal screenings, point-of-care testing, phlebotomy, EKGs, and other approved diagnostic procedures as needed. Participates in pre-visit planning to ensure care gap closure and optimize patient encounters. Engages in process improvement by developing and implementing quality improvement initiatives. Reviews medical records and other clinical documentation; conducts audits to ensure compliance and quality of care. Assists with data collection, health outcomes reporting, clinical audits, and program evaluations. Performs other related duties as assigned to support clinic operations and patient care.
