SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED
· Minimum of 1-2 years relevant experience in a community setting working with vulnerable populations.
· Experience working with Refugees and surrounding community.
· Demonstrated skills and experience in working with African Canadian and Caribbean populations, in addition to lived experience as a member of the African Canadian and Caribbean community.
· Demonstrated ability to work with sensitivity and compassion to deal with vulnerable populations.
· In-depth understanding of community resources and programs.
· Strong commitment to health prevention and client education programs to support clients throughout their lifespan.
· Knowledge of health care services and system navigation.
· Exceptional customer service skills to work with internal and external clients, community partners and stakeholders.
· Demonstrated ability to work independently as well as part of an inter-professional collaborative team environment.
· Excellent interpersonal, presentation, verbal, and written communication skills.
· Able to set clear goals and execute projects from start to finish with a commitment to quality.
· Uses a critical inquiry process to support professional judgement and reasoned decision making in day-to-day activities.
· Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation
· Bilingual (English/French/Arabic/Spanish) language skills an asset.
· Support programs with daily travel across Windsor and Essex County as required.
EDUCATION AND CERTIFICATE REQUIREMENTS
· College diploma or undergraduate degree in a relevant discipline from a recognized institution, or equivalent work experience and related certifications
· Language Interpreter Certification or equivalent an asset
· Current Basic Rescuer (C) certificate recognized by the Heart and Stroke Foundation of Ontario an asset.
This position will support the mission, vision and values of the Windsor Essex Ontario Health Team, weCHC and the work of the Refugee Health and Other Priority Populations – Coordinated Access Program. This position will report to the Director, Clinical Practice to support safety, quality care of clients/patients. The Clinical Service Coordinatorwill:
· Act as a resource to and advocate for Refugees and other priority populations (individuals, groups, and the community).
· Establish and maintain partnerships with other community-based organizations/resources.
· Build professional, productive, and positive working relationships both internally and externally to support the coordinated access pathway for this program.
· Participate in program development and adaptation.
· Protect the confidentiality, privacy and security of client and corporate information.
· Support the inter-professional/team with program areas, supplies and equipment ensuring items are safe and properly stored/maintained.
· Field calls from both Refugee Health and Black Health Coordinated Access Programs.
· Schedule clients with the appropriate teams and providers.
· System navigation for weCHC services and external services appropriate to client needs
· Process external referrals to mental health services and social service agencies.
· Work with the client to support accessing services outside the weCHC team.
· Discharge clients to primary care internally or externally.
· Act as a liaison for client and community connection.
· Respect the cultural diversity of the community and weCHC, ensuring an inclusive and welcoming climate for all.
· Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
