Overview:
The Clinical Services Coordinator II collaborates with clinical and non-clinical staff to facilitate the provision of coordinated interdisciplinary mental healthcare while meeting the needs of the patients/families served. This position is responsible for the efficient coordination of triage, acceptance, and case placement of referred patients. The incumbent will be the first point of contact for families and will coordinate needs prior to the initiation of services are initiated.
Responsibilities:
1. Triage referrals for Center for Developmental Behavioral Health (DBH) services by reviewing documents obtained from patients/families ensuring that the referrals meet the department’s criteria.
2. Monitor insurance benefit thresholds and facilitate the acquisition of authorizations by working with the Patient Access department. Maintains and stays abreast of all insurance requirements as they relate to the collection of accurate information.
3. Collect information from referring providers, patients/families, and medical records. Determine the appropriateness of fit and assignment and notify existing and new Institute providers of the shared case assignments.
4. Monitor and maintain various Epic Work Ques and related reports as needed.
5. Maintain full caseloads of clinicians within DBH by reviewing schedules to determine when to schedule additional evaluations.
6. Maintain regular contact with attending physicians to ensure that fellows are assigned appropriate types of cases.
7. Assist providers in transferring patients to higher levels of care as needed.
8. Assist sub-clinics and teams with triaging for clinic population and other special projects.
9. Lead the team during code green alerts.
10. Participate in training to develop clinical knowledge including mental health diagnoses, treatment models, and other therapeutic modalities.

Must obtain and maintain certification/training in the approved crisis response program as required by departmental standards.
