EDUCATION AND EXPERIENCE

• Current RN license in state of practice (Ohio)
• Current driver’s license with positive driving record required; ability to meet requirements of insurance coverage, required
• 4 - 6 years relevant experience
• Bachelors degree
• Basic computer skills and proficiency in MS Word, Outlook andExcel
• Demonstrated experience managing to data driven outcomesand process improvement
• Experience in healthcare performance coaching
• Current CRP certification
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
  Travel: Up to 75% depending on market Nights/Weekends: May include overnight travel
