Barnes-Jewish Hospital's staff is composed of full-time academic faculty and community physicians of Washington University School of Medicine, supported by a house staff of residents, interns, fellows and other medical professionals. Recognizing its excellence in nursing care, Barnes-Jewish Hospital was the first adult hospital in Missouri to be certified as a Magnet Hospital by the American Nurses Credentialing Center. The CM Services reports through the division of Patient Care Services lead by the Chief Nursing Executive, Vice President of Patient Care Services. The CM Service is comprised of three departments: Case Management, Utilization Management, and Social Work. Through the Triad Model of Care Delivery, the objectives of the CM Services are for all patients to have access to healthcare through the BJH continuum and appropriate, safe discharge plans. The SW department is open 24 hours/day while the CM department is open 20 hours/day. Both departments provide coverage throughout the hospital including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management. The Service employs registered nurses, social workers, a license practical nurse, a customer service representative, and administrative assistants. Preferred Qualifications Role Purpose Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning. Responsibilities Conducts screening and assessment activities to meet the needs of the discharged patient. Applies assessment findings to prepare a patient-centered plan of care. Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
