Responsibilities

JOB TITLE: Clinical Social Worker

About this role: The Clinical Social Worker will work with clients in specialized placement settings. The objective of this role is to ensure continuity of care and to translate community treatment plan goals into the residential placement setting. This role will include the assessment, development, and implementation of treatment plans, in coordination with other key stakeholders using a Trauma Informed approach and drawing from evidence informed and best practices in mental health care. This role will support stabilization, provide direct mental health intervention and crisis response. The Clinical Social Worker will work both independently and as a member of a collaborative multidisciplinary team. They will support the clients directly as well as the Specialized Community Support Workers providing care in the placement.

The Clinical Social Worker will be responsible for

  • Assessment, development, and facilitation of therapeutic interventions for clients within the placement setting, in collaboration with other members of the client’s care team
  • Presenting client care recommendations to inter-disciplinary teams, and working collaboratively with all stakeholders to develop individuals care plans for clients
  • Coordinating services and advocating for clients
  • Providing mental health intervention, including stabilization support and crisis intervention support
  • Maintaining clinical records and documentations
