The Child Center Residential Treatment Facility (RTF)
The RTF is a coeducational program in a warm, supportive environment that serves young people ages 12-18 who have had multiple psychiatric hospital placements or have been involved with the juvenile justice system. It is the only downstate RTF in the state that is a dual NYS Office of Mental Health/Office of Children and Family Services facility. We never consider it too late to give young people the help they need, and we are pleased to partner with these two agencies to ensure the young people in our care have the skills and support they need to achieve long-term success.
Reflecting The Child Center’s dedication to comprehensive care, the RTF is staffed by clinical coordinators, psychiatric providers, a pediatrician, and 24-hour nursing staff, 24 hour youth advocates. Supports are individualized and tailored to the needs of the young people.
PRIMARY FUNCTION: The Clinical Supervisor for Transitional Services is responsible for overseeing the engagement and support of RTF youth and families from entry through post-discharge (aftercare program) from the Residential Treatment Facility (RTF). The Clinical Supervisor works hand in hand with the Clinical Supervisor for Therapy Services to bring the family into the center of the treatment and discharge process and secure all entitlements, resources and supports for youth and families necessary to ensure positive outcomes post discharge from the RTF.

EDUCATION and/or EXPERIENCE

  • Master’s degree in human services field with at least 2 years of experience providing direct services to or linking children with SED to services
  • Supervisory and or managerial experience of at least 2 years in the social service sector
Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Responsible for the development, implementation and enhancement of structures and procedures of the Transitional and Permanency programs within the RTF with a goal of decreasing length of stay and minimal re-referrals for those in the program and providing greater access to RTF treatment for those in the community.
  • Provide ongoing clinical supervision and support to the transitional and family support teams of the RTF, assessing and addressing rehabilitative and clinical competencies with a special eye towards creating and strengthening relationships, healing family dynamics, educational success and positive socialization needs.
  • Oversee the teams’ assessment of family strength & needs, development and implementation of plans to address barriers to successful reunification, including progressive home time plans, skill building/behavior management techniques, caregiver support, coach and linkage to necessary resources including those addressing social determinants of health
  • Establish and lead joint meetings with TCCNY community-based programs to ensure efficient and effective supports for families, including but not limited to Health Homes, CFTSS, and Benefit Access
  • Ensure intensive community based, hands-on work with youth and discharge resources/families that leads to successful transitions into the community with a focus on integrating skills learned in the RTF into the discharge setting
  • Participate in clinical and treatment meetings including, but not limited to, LOS meetings, pre-admissions, CTPs and discharge conferences. Present weekly updates at Administration Meetings for the Transitional Coordinator Team
  • Establish and maintain collaborative relationships with, and current working knowledge of, local resources, as well as their required referral procedures and contacts
  • Support teams’ ability to effectively and efficiently navigate and communicate with systems impacting youth and families including, but not limited to, OCFS, DSS/ACS, DOE, Housing, Medicaid, etc.
  • Ensures all necessary school supports and needs are met, maintaining regular contact with the RTF in-house D75 school system
  • Work with Business Director to design, implement and monitor utilization of transitional/wrap-around funds for clients following OMH policies, ensuring they are utilized for their intended purposes
  • Ensure compliance with all OMH regulations and guidelines
  • Provide Transitional Coordinator services to a caseload as required, and upon any TC vacancies
  • Other tasks as required

SUPERVISORY RESPONSIBILITIES:

  • Backup to Director of Integrated Clinical Services
  • Carries out supervisory responsibilities for Transitional Coordinators, Permanency-Family Connections Specialist, and Intake Specialist in accordance with agency, government, licensing and other pertinent regulations, law and requirements
  • Provides supervision, assesses and addresses teams’ competence and compliance with documentation requirements via methods including, but not limited to, chart reviews, participation in community visits, and collaboration with Quality Specialist ensuring client and family care is consistent with best practice standards, policies and procedures
  • Manages the overall direction, coordination and evaluation of the Transitional, Aftercare and Family Support Entities of the RTF Program and Processes, specializing in community referrals
  • Serves as a critical member of the RTF Management team.
