RN- Clinical Support In office role 32 - 40 hrs. per week. Shift Option: 7:00am - 4:00pm, 8am - 5pm or 9am - 6pm No holidays/weekends FUNCTIONS: Triages calls assessing pain and symptoms, physical needs and disease progression Provides direction to the caller or coordinates necessary follow-up, prioritizing for those with uncontrolled pain/symptoms and imminent death Provides anticipatory guidance and education for disease progression and end of life care. Utilizes patient teaching tools and handbooks as reference. Assists with telephonic assessment and coordination of inpatient center transfers Obtains refills, renewals, new and revised orders for medication and treatments Supports the initiation of symptom management orders through timely notification to contracted pharmacy Updates the clinical record with new orders, medications and treatments, and communicates to team. Anticipates medication, DME and supply needs and bundles orders. Serves as a resource and mentor to hospice clinical staff Qualifications Registered Nurse licensed in Pennsylvania Associates in nursing required Minimum of two years medi­cal/surgical, home health, or relevant experience required Must be able to complete 5- 6 weeks of a FT (40 hr./wk.) orientation.
