An exciting opportunity has arisen for a Clinical Triage Co-ordinator to join the South West Interdisciplinary Team in the provision of Immediate and Planned Care to patients in the community. The multidisciplinary teams provide nursing and therapy services offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential and functional independence. Your role will include dealing with referrals to the nursing and therapy team, general administration work & supporting the Team Leaders.

To deliver call handling and support to the triage function of the community nursing and therapy team.
To support the administration functions of the community nursing and therapy team.
To work independently under the leadership of the team leads, therapists and registered nurses, working within Trust policies, procedures, and standard operating procedures.
To communicate effectively, with sensitivity and compassion, and to manage relationships with people, making reasonable adjustments where necessary.
