Description:
We are seeking a dedicated and experienced Program Manager to oversee the administrative functions of a five-county care coordination program. The ideal candidate will manage clinical supervisors, trainers, and other staff, ensuring effective program implementation and community outreach.
Requirements:

• Plans, coordinates, and directs the operation and services in accordance with established policies.
• Maintains administrative oversight of supervisors and trainers.
• Supervises, hires, trains, and evaluates the performance of supervisory personnel within the division.
• Responsible for hiring all CareLink staff.
• Monitors budget expenditures to assure cost-effectiveness.
• Identifies eligible members for the program.
• Manages outreach to community stakeholders.
• Provides education to community agencies.
• Serves as Medicaid company liaison.
• Develops policies and procedures for the program.
• Perform quality improvement checks for all programs.
• Stay up to date with state regulations regarding health homes.
• Attend meetings at the state level and network with other health home programs.
• Maintain HIPPA and client confidentiality at all times.
• Regular and consistent attendance for the assigned work schedule is essential.
• Performs other duties as assigned
