  • Background in Nursing, Medical Social Work, Counselling, Psychology or Ethics is preferred.

  • Proficient in Microsoft Office applications (i.e., Word, Excel and PowerPoint).
  • Relevant/related working experience in acute/intermediate and long term healthcare sector is advantageous.
Responsibilities

The Program Coordinator will support the operations of Stroke Rehabilitation Ecosystem, with the leadership of the Program Clinical Leads in the coordination, implementation and evaluation works related to the ecosystem. This includes:

  • Review daily stroke admissions to identify and engage patients eligible for the program.
  • Collate and track data on patient’s progress and assist with analysis of data.
  • Provide basic psychological interventions.
  • Assist to redirect patients to suitable service tiers should patients clinically deteriorate.
  • Ensure relationships with key agencies and community partners are maintained.
  • Implement workflows to ensure seamless referral and operations.
  • Assist in planning, implementation, and management of activities and projects that contribute towards service/quality/process improvement and patient care.
  • Assist in enhancement of service and quality of the ecosystem and its component services.
  • Arrange and support relevant meetings with internal, external stakeholders and partners.
  • Support progress updates to funding organisation.
  • Participates actively as a member in department, division, and wider hospital initiatives.
  • Any other matters as assigned by HOD/Supervisor and/or Program Clinical Leads.
