The Filipino Community Council of Victoria, Inc. (FCCVI) is a duly incorporated non-political, non-stock, non-profit, and benevolent community service organisation, founded in 1988.
FCCVI is recognised as the umbrella body of the Filipino organisations in Victoria. Its existence came about as a direct request from the Australian Government in the late 1980s through the then Minister of the Department of Immigration to be the primary contact in communicating and collaborating with individual Filipino organisations rapidly increasing at that time.
We are seeking a suitably qualified Community Home Support Program and Lifestyle Coordinator to work in our Brooklyn based office in Melbourne’s West. Knowledge of the Filipino culture and language is preferred along with previous experience in a similar role.

Responsibilities
  • Manage care plans,
  • Coordinate in-home and community-based services and activities for older adults, people with disabilities and their carers who are in receipt of CHSP and (HACC PYP) services.
  • Liaise with service providers, community stakeholders, funding bodies and relevant networks to identify partnership arrangements to increase social support options to meet the goals of clients.
  • Coordinate and facilitate the delivery of a program of social support activities designed to meet the individual goals of clients.
  • Supervise, support and roster social support group assistants and volunteers.
  • Organise and oversee the safe transport for Social Support Group.
  • Work with the Home Care Package program to ensure that Social Support clients retain FCCVI as their service provider as they transition through different levels of home care support.
  • Promote FCCVI and its services within the community.
  • Administer services in accordance with legislative and contractual requirements.
  • Maintain computer based and paper file documentation of client information.
  • Comply with organisational safety, quality & risk policies and guidelines.
  • Comply with FCCVI continuing professional development requirements.
  • Participate in staff development opportunities as planned and required.
  • Actively participate in annual performance development reviews.
  • Attend meetings and In-Service training as required
