Description
The Community Aide II is an entry level position responsible for assisting with the assessment, education and rehabilitation of consumers participating in the program. Our residential programs embrace a Housing First, Harm Reduction philosophy to meet our participants where they are. We are widely recognized across Allegheny County for providing the highest quality of support and service to our participants, and serve individuals diagnosed with behavioral health and substance use disorder. Responsibilities: Collaboration & Communication Meet regularly with Vocational Counselor, Program Coordinator, and Psychiatric Rehabilitation Staff to discuss consumer progress and service planning. Maintain ongoing communication with supervisors and staff regarding group and individual activities. Report clinical and non-clinical concerns to appropriate administrative personnel. Attend staff meetings, training sessions, and supervisory meetings; contribute to discussions on consumer involvement. Consumer Support & Engagement Work directly with consumers in various settings to provide orientation, monitor progress, assess skills, and offer continuous feedback. Engage consumers effectively, modeling appropriate interactions and providing 1-on-1 or group support. Identify individual learning needs in collaboration with the Program Coordinator. Program Development & Evaluation Participate in planning, implementing, and evaluating rehabilitation services and recovery programs. Collaborate on the development of individualized Rehabilitation Plans based on ongoing needs. Documentation & Safety Accurately document goal progress, services provided, observations, interventions, and consumer responses. Apply and uphold safety precautions in all environments. High school diploma or GED required. Prior human services experience preferred. May in the course of their duties be in rooms where medications are stored or may transport medications when they transport patients. Transportation of medication is limited from one department to another. Licensure, Certifications, and Clearances: Valid Pennsylvania drivers license and access to reliable transportation preferred.
