We are looking for an energetic and innovative AHP rehabilitation support worker to join our community team. If you are motivated and passionate about person-centred services which support the wellbeing of people to live and age well in communities, then get in touch. The post holder will work across the community linking with AHP based teams.
This post holder will be working primarily with people experiencing frailty, have fallen or at risk of falls. As part of the therapy team you will develop a delegated caseload with support from the AHP and be expected to work independently, assessing the patient’s needs, providing a prescribed exercise and/or activity program and liaising with onward health or third sector agencies all without direct supervision. This includes assessing, progressing, reassessing and discharging patients within the scope of practice. You will have excellent communication and interpersonal skills to complete clinical and non clinical duties together with service development activities.
Ongoing personal and professional development is essential to provide a quality patient centred service and supported through CPD, supervision/appraisal and training opportunities.
