What You’ll Do - As a Community Care Coordinator, you’ll serve as the face of Health at Home within the community—connecting with physicians, hospitals, senior living communities, rehab centers, case managers and our community. Qualifications What We’re Looking For - 1+ years of experience in marketing or sales (home health preferred), with the skills to build relationships, drive referrals, and deliver results A strong communicator and networker who thrives in the field Passion for serving seniors and individuals in need of in-home care Valid driver’s license & reliable transportation High School Diploma required, Bachelor’s Degree in Marketing, Business, or a health-related field preferred Clinical background (RN, LPN, PT, etc.) a plus—but not required Why Join Health at Home?
