We have an opening for a Community Care Hub Specialist in our (hybrid) Tacoma office. If you are looking for an excellent career opportunity at a successful, mission- and values-driven organization, with an amazing team, and a hybrid work schedule, this is a unique opportunity!
As a Community Care Hub Specialist, you will be responsible for developing and delivering training to Community Based Workers, providing shared learning and peer networking opportunities, and providing network partners with support to meet their contracted milestones and expectations. You’ll also assist the operations team by ensuring that referrals and case assignments are entered, processed, and proceed through care coordination workflows as specified. In times of high referral numbers, you may also provide direct client support to community members and clients in the form of short-term resource navigation and resource brokering.
