We're looking for a proactive Community Care Supervisor to join our dynamic Reablement team. In this key role, you'll provide vital support and lead a friendly team of Support Workers to deliver the best possible care services. If you're looking to grow your career within a supportive and fast-paced environment, we’d love to hear from you.   Role Purpose: Planning safe and efficient work schedules for the Care Team; giving all on-the-job instruction and supervision to all members of the Support Team, supervising delivery of care/reablement, being the main point of contact with partner agencies to process and set up new referrals and ensuring detailed reablement/care plans meet the individual needs of service users and that the quality of care meets the highest standards. The emphasis being on reablement and maximising people’s independence to achieve person centred goals and outcomes that maximise their ability to support themselves as far as is safe and possible.   What you'll be doing:    * To hold responsibility for oversight of work schedules planned for Support workers & Senior Support workers and to ensure as far as possible these remain optimised at all times.  * To be responsible for overseeing the day-to-day operation of the services and for all aspects of staff support, including team performance, staff development and attendance management * To support the offer of TECH enabled approach to care  * To work collaboratively and in partnership with emerging models of care, through an integration and partnership approach, with Health and voluntary organisation partnerships, to deliver timely and person centred levels of care and support whilst optimising individuals outcome across Cornwall,  * Liaise with Health and social care partners to process referrals in a timely manner and facilitate safe interventions of support whether from Hospital or Home starts.  * Supervise and monitor allocated Community Support Workers and take appropriate action as required including appraisals and regular 1:1 reviews with team members  * Participate in national and local development programmes by carrying out the Care Certificate training for new staff and assessing competence * Deliver ongoing local formal and practical training to all team members to include client specific topics as required by Care Quality Commission (CQC) and ensure records are maintained  * Mentor and/or coach Support Workers and less experienced Community Care Supervisors to meet the required standards. Participate in the interviewing, selection and induction of new support workers, in conjunction with the Registered Manager  * Carry out initial visits to service users to produce detailed reablement / support plans and monitor and review those plans on a regular basis and as service user needs change ensuring that visit times are amended to reflect changes in abilities and/or circumstances. * To complete Starta forms and assist with the provision of appropriate ongoing services where appropriate.  * To undertake the relevant training and be responsible for assessing for and providing low level pieces of equipment which will enable customers to remain safe and independent at home.    * Carry out detailed provider risk assessments and moving and handling risk assessments to minimise the areas of risk to the service user, staff, service providers and the general public, and to carry out regular reviews, ensuring information is shared with relevant parties * Monitor the service provided to service users according to published Quality Standards ensuring regular feedback of appropriate information to Registered Managers and Commissioners, thus ensuring the safety and wellbeing of the service user and the staff involved * Receive and investigate initial complaints, accidents and incidents according to procedure and ensure that any safeguarding issues are dealt with expeditiously  * Maintain and monitor records in relation to annual leave, sickness, mileage claims, reablement plans, service user visit records, risk assessments, etc. using relevant systems * Chair and minute team meetings and participate in local, district and countywide meetings, multi– disciplinary meetings, hub meetings and other meetings relevant to the post  * To be aware of all relevant policies and procedures and take responsibility for ensuring this knowledge remains up to date and is shared with Support workers.  * Maintain open effective communication channels within the service, organisationally and with other agencies * Attend training courses as required and maintain own professional development and competency including having responsibility for own self development on a continual basis * Undertake the duties of the Support Worker as required in order to maintain personal skills in delivering reablement /support  * Work at all times within CQC required standards and the refreshed management common induction standards and codes of practice.  * Operate with due regard to Corserv’s confidentiality policies, the Data Protection Act and best practice and work at all times within the requirements of Health and Safety legislation * Where identified Participate in the Out of Hours Duty System to support staff and service outside of office hours * Carry out any other duties related to the post at the direction of the Community Services Management Team, which could include those duties listed outside of the local team, i.e. cover across the district or services as required and appropriate to the grading of the post.  * Ensure a positive and flexible approach to the variety of tasks and work patterns within the role as the service develops to enable effective and appropriate staffing levels to meet demand including working weekends and evenings or early mornings.  * To operate in accordance with the diverse needs of the community to ensure equal access to services * Be responsible for self and others continuous development and to undertake relevant training as required.   About you:  Minimum level 2 qualification in Health and Social Care or equivalent. Experience in providing direct care in a work setting. Supervisory / Line Management qualification and/or demonstrable experience of leading a team in frontline care including scheduling a programme of work.
