JOB SUMMARY

We are seeking a compassionate and dedicated Community Care Worker to join our team. QUALIFICATIONS

  • Excellent communication skills, both verbal and written.

  • Ability to work independently as well as part of a team.

Responsibilities
  • Provide personal care and support to clients, including assistance with daily living activities such as bathing, dressing, and meal preparation.
  • Mentor clients by encouraging their personal development and promoting independence in their daily routines.
  • Supervise clients during various activities to ensure their safety and well-being.
  • Develop and maintain positive relationships with clients, their families, and other healthcare professionals.
  • Assist with medication management as required, ensuring adherence to prescribed regimens.
  • Document client progress and report any changes in condition to the appropriate healthcare personnel.
  • Participate in training sessions and team meetings to enhance skills and knowledge related to community care.
