Job DetailsLevel: EntryJob Location: Hartford, CT 06132Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $21.57 - $21.57 HourlyTravel Percentage: Up to 25%Job Category: Nonprofit - Social ServicesPOSITION SCOPE: Work in partnership with clients to provide counseling, case management and employment services which develop the independent living skills necessary for community living. A focus on interpersonal, vocational and decision-making skills and social supports that result in community integration, employment and increased ability to live independently, free of the criminal justice system. ESSENTIAL RESPONSIBILITIES Maintain caseload as assigned by supervisor. Caseload may exceed 28 clients per staff member at any time. Complete concurrent documentation in the field and in the office on all client and client related contacts. Documentation should be completed no later than 24 hours after the contact. Transport clients in personal vehicle to and from client appointments, to and from DOC and to resource as assigned by Project Manager. Supervise and collect urine samples from clients to be submitted for analysis. Manage documentation and tracking of urine samples. Complete assessments within 14 days of intake. Provide 24 hour on call crisis intervention after hours as assigned by Project Manager. Facilitate client obtaining groceries and essential items through the system chosen by the agency. Staff may be requested to assist clients in bringing these items to the assigned apartment, with or without the client. Work alone in 24/7 residential setting with clients and in clients assigned apartments. Ability to work alone conducting inspections as determined by Project Manager for community safety purposes. Travel to CT DOC institutions to screen potential clients. Work with each person on caseload to conduct an assessment and develop a case management plan which identifies needs and the actions necessary to meet the needs and which focuses on increasing the client’s ability to live independently, including clear and measurable goals for attainment. Assistance to obtain offender identification such as birth certificate and social security card. Provide assistance with obtaining, maintaining safe affordable housing and employment, including interventions with landlords and employers. Qualifications Bachelors degree plus a minimum two years experience in criminal justice field or related field. Training in business or evidence of satisfactory work experience. Ability to work with clients experiencing mental illness, HIV/AIDS, chemical addictions, incarceration and homelessness. Knowledge of assessment and crisis intervention methods, criminogenic needs and principles, CTDOC Fidelity Model, community resources, recovery planning and employment services Demonstrated group and individual counseling and educational skills. Knowledge and proficiency in using computer applications, including standard Windows applications. Good interpersonal, organizational, oral and written communication skills. Fluency in Spanish is desirable. Demonstrated ability to work in partnership with staff, clients and the community and autonomously in all community and agency settings. Must be able to lift over 50 pounds and carry objects. Possess and maintains a valid Drivers’ License, reliable vehicle, current registration and insurance. KEY COMPETENCIES Demonstrates organizational, written/composition skills and verbal skills. Demonstrates computer skills with proficiency utilizing Microsoft Office, Electronic Health Record, filing skills and other administrative skills. Demonstrates effective interpersonal skills, including professional demeanor and positive attitude toward co-workers. Demonstrates ability to maintain confidentiality of information concerning agency matters. Knowledge of criminogenic needs and principles, CT DOC, employment services, and community resources including but not limited to SSA, SSDI, SSI, SAGA, DSS. Knowledge of substance abuse, drug usage, sex offenders and state and local laws. Available for flexible shifts as needed Staff must have the ability to receive DOC approved access to the prison system
