The Role You Play as a Case Manager As a Community Case Manager, you’ll provide person-centered support to children, teens, and adults by meeting them in the community and helping them build skills, manage symptoms, and connect to needed resources. You’ll collaborate closely with families, treatment teams, and community partners. Some evening flexibility may be required, but you set your own schedule. The Day-to-Day Provide community-based services in homes, schools, workplaces, shelters, and other natural settings. Conduct strengths assessments and develop individualized goals with clients and treatment teams. Offer therapeutic interventions focused on symptom management, coping skills, and daily functioning. Connect clients and families with needed resources, benefits, and supports across the community. Maintain accurate, timely documentation and manage your schedule in the electronic health record.
