COMMUNITY CASEWORKER

For over 40 years Second Chance Headway Centre has provided specialist services and support for people with Acquired Brain Injury, their families and carers. Alongside our other services the Centre offers information, advice and advocacy to people affected by brain injury – including survivors, family members, friends and unpaid carers. Over the last five years we have developed our casework services and now offer a range of support to people affected by brain injury including information provision, advice and advocacy. The casework service also aims to provide specialist support, training and advice to partner agencies and to support with complex case management. Our Casework team provide family members with support and information about what has happened to their relative and to help them navigate the health & social care system, provide them with information on entitlements and signpost them to other relevant services.
We are looking for an enthusiastic, reliable and confident person to join our caseworker service as a community caseworker. The ideal candidate will provide face to face advice and support to people with brain injury and their families, across West Yorkshire. Applicants need to have a minimum of 12 months experience in a health or social care setting ideally in a similar role. The successful candidate will be responsible for planning and negotiating a range of appropriate services and interventions that meet the need of service users. A central part of this will be in supporting clients to access statutory services and timely rehabilitation where indicated.
The successful candidate will be working as part of a wider MDT at Second Chance and will have the opportunity to support partner agencies with complex case management. The role includes being involved in delivering training to partner agencies. The successful candidate may be requested to work within the other services at Second Chance depending on whole service demands.
As this is a developing service the successful candidate will be working with the casework team to develop the service and to complete research and evaluation of the project.
Supervision and professional development will be supported.

EXPERIENCE

  1. Experience of working with adults who have physical, cognitive, psychological, or learning needs.
  2. Of working within a person centred/Strengths based model of care.
  3. Producing written records and reports to a high standard for a variety of purposes with language suited to function

SKILLS, KNOWLEDGE AND ABILITIES

  2. Understanding of the roles and responsibilities of statutory and voluntary agencies.
  3. Demonstrate an understanding of the importance of confidentiality and the needs and wishes of individuals, families, and carers as appropriate
  4. Demonstrate an awareness of needs and safeguarding and risk issues associated with vulnerable people. An awareness of Safeguarding frameworks and legislation.
  5. Excellent verbal and written communication skills

Responsibilities

CORE RESPONSIBILITIES:

  1. Support clients with acquired brain injuries and their families, to maximise or maintain their autonomy and independence in their home environment and their community.
  2. To carry a caseload of community cases and, working in a collaborative way with clients, undertake assessment of service users and their support needs using a person centred, strengths-based approach.
  3. Responsible for planning, identifying, and negotiating a range of appropriate services and interventions that meet the needs of service users.
  4. Support clients to access statutory services and timely rehabilitation where indicated. Make appropriate referrals to other SCHC services, social services, community rehabilitation teams, other voluntary services and / or any other relevant support services.
  5. Support clients to identify and access community resources: including educational, social, leisure, physical and vocational activities.
  6. Provide ongoing support to clients, carer and families as needed.
  7. Manage and monitor a workload of diverse cases within the parameters of the agreed service description together with professional guidance and support from the Service Manager. Very complex cases may be co- worked with Service Manager
  8. Support clients in accessing their statutory rights and maximising their income through benefit entitlement and budgeting. Support clients to complete relevant statutory paperwork.
  9. Provide appropriate and relevant advice, information, and advocacy, to clients and their families.
  10. Engage interpreters, signers and advocates (as necessary to ensure all clients have access to the same level of support).
  11. Provide specialist advice and support to partner agencies
  12. Organise work activities independently taking account on the need to prioritise tasks and responsibility.
  13. Maintain and update case notes and other records, write reports as required.
  14. Appropriately participate in meetings and relevant forums in line with service aims and protocols
  15. Contribute to the ongoing research, development, and evaluation of the service
  16. Be compliant with relevant governance, policy, and procedures
  17. Develop and maintain effecting working relationships with other professionals within SCHC and with partner agencies to ensure integrated, holistic and multidisciplinary approach to the care management and safeguarding of service users.
  18. Be responsible for understanding and applying all relevant practice standards, policies, and procedures. Maintain an awareness of related legislation. Disseminate knowledge and contribute to the delivery of a high standard of service.
  19. Contribute to the delivery of training to partner agencies and the publicising/promotion of the service.
  20. Support with students and offer support to other team members and or volunteers.
  21. Seek and engage in regular supervision with the Community Case Worker Service Manager and attend team meetings, as required.
  22. The successful candidate may be requested to work within the other services at Second Chance depending on whole service demands.
  23. Report to the Community Case Worker Service Manager.
  24. Any other duties that may reasonably be required.
