Due to an expansion in our service we have exciting opportunities to support the Children’s Community Nursing Service in providing respite care, supporting outpatients clinics and special needs schools and some aspects of clinical care under the guidance of the registered nurse.
To work as part of the Children’s Community Nursing Team by supporting the registered nurse in delivering a high standard of care to the child/ young person and their family.
To work as part of the nursing and multi-disciplinary team in the child’s own home and other establishments under the direction of a Community Children’s Nursing Borough Manager.
Opportunities have arisen for enthusiastic HCSWs to join the Childrens Community Nursing Service (CCNS) in ABUHB. We are looking for motivated individuals who enjoy working with children in a variety of settings and environments. The individuals must have the pre requisite Level 3 qualification in health and social care or childcare.
To work as part of the CCNS by supporting the registered nurse in delivering a high standard of care to the child / young person and their family.
All necessary training will be given and the successful candidates will be supported to develop their knowledge, experience and skills. We also encourage wider development and career progression is encouraged.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Continuing Care
The team are responsible for providing packages of care to children following a recommendation of hours approved by the funding panel.
Care Closer to Home
The CCN team provide a clinical service, the aim of the service is to provide a skilled nursing resource for children, young people and their families, reducing hospital admissions where possible and facilitating early discharge. The team accept a number of referrals from hospitals and community agencies.
Paediatric Outpatient
The CCN team is also responsible for managing and staffing all paediatric outpatient clinics across ABUHB.

There are four special needs schools within Gwent supported by the CCN tea
