Achieve with us.  Position:   Life Coach VI / Community Coach  Salary:        $20.18 per hour Job Type:   Full-time (40 hours weekly)  Schedule:  Tuesday - Saturday, may include evenings & weekends. Locations:  Walton  Responsibilities:  Support people to live personally fulfilling lives and achieve their person centered valued outcomes through a positive and proactive approach.  Other duties:  * Provide training and support to ensure participants learn necessary skills for successful employment and community life. * Assess individual’s strengths and interests to develop and implement person centered goal plans. * Assess the job and complete a task analysis. * Perform training on adaptations and specialized interventions apart from others in setting to avoid drawing negative attention to people supported. * Work cooperatively with job site supervisor and maintain ongoing, open communications. * Maintain records appropriate to the program including but not limited to  1) Daily task analysis record   2) Job coach monthly progress reports for  ACCES-VR   3) Daily/weekly log notes  4) Mileage sheets. * Submit billing documentation accurately and timely. * Excellent Communication skills * Strong attention to detail * Innovative problem solving skills * Entering/uploading data in Medisked * Provide support in other areas of the agency * Passion for helping others, and the ability to build relationships.    Qualifications: * High School Diploma or equivalent, plus two year experience preferred * Ability to lift 50 lbs. * Ability to prioritize, multitask, and manage busy schedules/deadlines * Ability to pass a Criminal Background Investigation * Willingness to obtain and maintain First Aid and CPR Certification * Willing to work nights and weekends * NYS Driver License * Good verbal & written communication skills * Willing and eager to learn as things arise. * To be flexible, ability to think on the moment, easily create a back-up plan, work without direct supervision.     Equal Opportunity/Affirmative Action Employer female/minority/disability/vet/  www.delarc.org
